Please plan to arrive in our office approximately 15 minutes before your appointment to fill out your new patient paperwork.

Also, please bring the following:

            - ID card

            - Insurance card (for insured patients only)

            - Referral forms (Tricare and Medicare/Medicaid)

            - Privacy Policy  

            - Patients Demographic  

            - General Release Authorization 

            - Compliance Assurance Notification

Returning patients:

If you have recently moved or changed insurance, please bring new ID and insurance card with you to your next appointment.

Please note that it is our policy to not disclose ANY information about you or your health records unless you specifically instruct us to do so in writing. If you would like us to leave phone messages with your spouse or other members of your household, please fill out a disclosure form.
